The Garden City Family Health Team offers our patients a team approach to health care. Our team includes patients, family doctors, social workers, dietitians, nurse practitioners, and a psychiatrist. As a team, we work together to come up with the best possible plan for the health and wellness of our patients.
 

OUR VISION

Our vision is to work with our patients and families towards optimizing
their health status and well being. Our focus is on health care, health promotion, disease prevention and chronic disease management.

LEARNERS

We are a teaching facility. Throughout the year, we welcome medical students and students in other fields including nurse practitioner students, social work students and dietitian students.

COMPUTERIZED HEALTH RECORDS

Part of our dedication to the well-being of our patients is having all of our patient records in electronic format. Gone are the days of paper files. All of our patient records are on computer. Our patient records are securely kept and protected.

ENROLMENT: JOINING THE TEAM

Our patients are a big part of our team. We want to work with our patients and families so that we can set goals together to promote health and well being. We encourage patients to enroll as members of our team. Enrolment gives patients access to all of our services and allows us to share necessary records with any team member that is participating in a patient’s care. Enrolment is optional. Patients who choose not to enroll are not eligible for our team services but still have a family doctor.

We are honoured to have a team of dedicated members who are committed to work with each other and with our patients.
