About the Program
 
When you visit the Garden City Diabetes Care Team you receive the services of trained health care providers who specialize in diabetes prevention, care and management. We currently offer a diabetes education program that is open to adults with diabetes in St Catharines.
 
In order to be seen within the program clients must be referred by their physician.

Self-referrals are accepted for the Niagara Region group education classes ONLY. The Poster includes the dates, times and locations for all open access classes.
 
 
Program Services
 
Through ongoing education, counseling and care the Garden City Diabetes Care Team offers:
 
  • Individual assessments to determine self-management goals
  • Options for either individual or group education on topics such as understanding of diabetes and how it affects your body, recommended targets for blood glucose control, guidelines to healthy living with diabetes and prevention of complications related to diabetes
  • Insulin initiation and follow-up for dosage management
  • Healthy eating for optimal nutrition
  • Exercise and ways to promote a healthy lifestyle
  • Cholesterol education classes to help with heart disease prevention
  • The importance of foot care
