Mission
The Ontario Ministry of Heath and Long-Term Care funds the Diabetes Education Program housed within the Garden City Family Health Team in support of the Ontario Government’s decision to make diabetes prevention and control a major health priority in our community. Operated by the Diabetes Care Team for all residents of St Catharines, it offers both education and self-management support to help you reduce your risk of complications from diabetes and help you manage the disease.
We are staffed with a diabetes registered nurse educator and registered dietitian educator who work with your primary health care providers (ie) doctor or nurse practitioners, in collaboration with community partners such as the local Canadian Diabetes Association.
The Greater St. Catharines Diabetes Care Team strives to provide knowledge; resources and self-management support to assist adults with diabetes live healthier lives in accordance with the Canadian Diabetes Association Clinical Practice Guidelines.
