1. Who is the Privacy Officer?Sandy Scapillati is the Privacy Officer for the Garden City Family Health Team.
2. How do I reach my doctor after 5 p.m. or on weekends?Your doctor is available Monday to Friday, from 9:00 a.m. to 5:00 p.m. If you need to talk to someone from your doctor’s office from 9:00 a.m. to 5:00 p.m. and you reach the answering service, leave your name and phone number with the answering service and ask the operator to contact your doctor. If your call can wait until the office re-opens, ask the operator what time the office opens and call back.
After 5:00 p.m. and on the weekends, there is always a doctor on call for enrolled patients. If you think you need to speak to the doctor on call regarding a medical problem that cannot wait until the next business day, leave your contact information with the operator and the doctor on call will return your call.
3. What is enrolment?Enrolment means signing up as a member of the Garden City Family Health Team. Only enrolled patients have access to our after hours services and special programs and services. Patients who choose not to enroll see their family doctor during regular office only and may not participate in any of our programs or services. Click here for more information on enrolment.
4. How do I know if I am enrolled?Enrolled patients fill in a white form that has yellow and pink pages attached. The form has sections for your name, address, phone number, health card number and permission to share your medical information with other members of the team. If you do not remember filling out an enrolment form, ask your doctor’s receptionist to check your enrolment status.
5. Can I enroll or un-enroll at any time?Even if you are not currently enrolled, you can ask your doctor’s office for an enrolment form. You can un-enroll at any time.
