Our team approach to health care means that we offer a variety of programs and services to patients who enroll with the Garden City Family Health Team.
What does it mean to enroll with the Garden City Family Health Team?
Because your records are confidential and cannot be shared without your permission, we need patients to enroll and give consent if they are going to use any of our services.
Enrolment involves filling out and signing a document with your contact information. Once you fill out and sign this page, you are eligible to use the services of the Garden City Family Health Team. We need you to fill out this document so that we have your permission to share parts of your medical records with other team members that you might see. Without your signed enrolment, team members who might need to see you cannot have access to your records.
Enrolment is consent for the Ministry of Health to release information to your doctor about preventive services (like a mammogram or colonoscopy) you may receive elsewhere.
What if I do enroll? What services and programs might I use if I do enroll?
Enrolling with the Garden City Family Health Team means that you want to be part of the team approach to health care. We have a variety of services and programs that we offer to our enrolled patients.
If you have not yet enrolled but want to sign up with the Garden City Family Health Team, drop in at your family doctor’s office during regular office hours and pick up a form. Please note: each person aged 16 and over in your household needs to fill out and sign a separate form.
Are there are restrictions for enrolled patients?
Enrolling with the Garden City Family Health Team means coming to us for your medical needs. Enrolled patients agree not to use walk-in clinics. We always have a doctor on call and offer after hours urgent care clinics. Click here to be linked to our after hours care brochure.
