We have gathered a team of health professionals who work together with patients and their families to come up with the best ways for our patients to live healthy lives.

Our Mission
The Garden City Family Health Team will provide comprehensive, compassionate and holistic primary health care to patients/families, which will improve their health and well being and recognize patient accountability. Utilizing the interdisciplinary team approach, and supported by an integral IT platform, the Family Health Team will focus on core primary care services, health promotion, disease prevention and chronic disease management. The Family Health Team will collaborate with other service providers and facilitate timely access to health services not available at the Garden City Family Health Team. The Family Health Team will work with educational institutions to expand the clinical teaching program for medical and healthcare students and enhance the learning environment for team members.
